GUIDE PROGRAM

GUIDE Program – Personalized Support for Families Facing Dementia

If you are caring for a loved one with Alzheimer's disease or another form of dementia, you know how difficult and emotional the journey can be. At Savannah Neurology Specialists, we see the incredible love and dedication it takes — and we want you to know that you don’t have to do it alone.

Starting July 1, we are honored to offer the GUIDE Program, a new initiative designed to provide hands-on, compassionate support for both individuals living with dementia and the people who care for them. This program is completely free for patients who have Traditional Medicare Part B.

Through the GUIDE Program, families are matched with a dedicated care navigator — a specially trained dementia expert, trained through Johns Hopkins University, who will walk beside you throughout your caregiving journey. Your care navigator will take time to understand your unique needs, offer emotional support, help you manage day-to-day challenges, and connect you with helpful resources and services as your loved one's condition evolves. One of the most valuable parts of this program is access to up to 80 hours of respite care — giving caregivers time to rest and recharge while knowing their loved one is in good hands.

If you're caring for someone with dementia and want to explore how the GUIDE Program could support your family, we encourage you to reach out. You can call Craniometrix at (912) 456-8858, or click the link below to schedule a call with a care team member who can walk you through the next steps.

We are here to help you feel more supported, informed, and empowered — every step of the way.
